Why does thickness matter more here?

Every layer between the heating element and the room is resistance. A quarter inch of dense felt plus a hand-knotted rug is a lot of resistance, and the client notices as a cold spot exactly where they put the seating group.

Is felt safe over radiant at all?

Yes, and it is the better choice. Felt breathes, so moisture moves rather than collecting against the floor. Solid synthetic and rubberized pads trap both heat and vapour.

Does anything change about the install?
