Why measure twice along the same side?
Because a 9x12 is rarely 9x12. Hand-knotted rugs come off the loom with real variance, and antique rugs have usually moved since.
Measure the width at both ends and the length at both edges. If they disagree, that difference is what you have to design around.
Which figure do you order to?
The larger one, every time. A pad slightly too big can be ripped down in seconds. A pad too small leaves the rug's edge unsupported, which is where rugs curl, trip people, and wear out first.
How do you handle it on site?
Lay the pad, set the rug, and walk all four edges. If the pad shows on the narrow end, rip one 2-inch strip off that side and re-check. One strip usually does it.
What about fringe?
Measure to the woven edge and exclude it. A pad running under fringe lifts it off the floor and makes a hand-knotted rug look propped rather than laid.
